What is Form 80?
Form 80 โ formally titled Personal Particulars for Assessment including Character Assessment โ is a form issued by the Australian Department of Home Affairs. It is used to assess the character of visa applicants and covers detailed personal history across 20 sections: identity, travel history, residential addresses, employment, education, family, criminal history, and more.
The form is 19 pages long and contains 54 individual questions. Some questions require a single response; others require complete chronological histories with no gaps. It is one of the most detailed character-assessment forms in any immigration system.
The official Form 80 PDF is available free of charge from the Department of Home Affairs at immi.homeaffairs.gov.au. You can also fill Form 80 online free using FormMate 80 and download a completed PDF.
Who needs Form 80?
The Department of Home Affairs requests Form 80 on a case-by-case basis. You are most likely to be asked to complete it if:
- You are 18 years of age or older
- You are applying for a visa that includes a character assessment
- You have lived in certain countries for 12 months or more in the past 10 years
- You have a criminal history or have previously been refused a visa
- Your case officer specifically requests it
Not every applicant is required to complete Form 80. The form is requested โ not automatically required โ and your case officer or ImmiAccount will indicate if it is needed.
Is Form 80 mandatory?
Form 80 is mandatory when the Department of Home Affairs requests it for your application. If requested, failure to provide a completed Form 80 will delay or stop processing of your visa application. If it has not been requested, you do not need to submit it unprompted.

Address history (Part F)
Part F requires every residential address since age 16, with no gaps. This is one of the most frequently incomplete sections. You must list even short stays โ student accommodation, temporary housing, periods living with family or friends. If there is a gap, explain it in Part T.
Employment history (Part K)
Part K requires a complete employment history with no gaps. This includes full-time, part-time, casual, self-employed, and contract work, as well as periods of unemployment and full-time study. Each entry requires the employer's name, address, and your position or role.
Travel history (Part I)
Part I requires all international travel in the past 10 years, including every country visited, approximate dates, and the purpose of travel. Transit stops longer than 24 hours must be included. See the Form 80 travel history guide for a detailed explanation.
Criminal history (Part O)
Part O asks about all criminal charges, convictions, and sentences in any country โ regardless of how minor, whether overseas, or whether considered "spent". This includes dismissed charges and fines. See the Form 80 criminal history guide for what must be disclosed.
Common mistakes when completing Form 80
- Gaps in residential history โ Every address since age 16 is required. Even one month missing triggers a request for additional information.
- Incomplete travel history โ Short trips, transit stops, and holidays are all required. Cross-reference passport stamps and booking confirmations.
- Omitting overseas criminal records โ Non-disclosure is treated more seriously than the underlying offence. When in doubt, disclose.
- Inconsistent dates โ Dates must be consistent with your main visa application and other documents.
- Leaving sections blank โ Sections that do not apply must be explicitly marked "No" or "None". Blank sections are treated as incomplete.
Form 80 vs Form 1221
Form 1221 (Additional Personal Particulars Information) is a shorter form used for certain temporary visa categories. Form 80 is more comprehensive and required for character assessment in most permanent and some temporary visa applications. See the detailed comparison: Form 80 vs Form 1221 โ which one do you need?
