Form 80 vs Form 1221: Which One Do You Need?

By FormMate 80  ยท  Updated June 2026  ยท  5 min read

Two of the Australian Department of Home Affairs' most commonly requested character-assessment documents are Form 80 and Form 1221. Both ask about your personal history, travel, and character โ€” but they are different forms used for different purposes, and confusing them is a surprisingly common mistake.

This guide explains what each form covers, when each is required, and what to do if you're not sure which one you need.

โš ๏ธ FormMate 80 is an independent tool and is not affiliated with the Australian Government or the Department of Home Affairs. Whether you need Form 80, Form 1221, or both is determined by your visa subclass and your case officer โ€” not by this guide.

What is Form 80?

Form 80 โ€” Personal Particulars for Assessment including Character Assessment โ€” is the Department's comprehensive character assessment tool. It spans 19 pages and 20 sections (Parts A through T), covering your entire personal history in detail.

Form 80 is the more substantial of the two forms. It is used when the Department needs a thorough picture of an applicant's background, typically for permanent residence applications and visa types with a significant character assessment component.

Key sections unique to Form 80 include:

  • Full residential history since age 16
  • Complete employment history
  • Military service in any country
  • Political and organisational memberships
  • Government positions held
  • Business and corporate associations

What is Form 1221?

Form 1221 โ€” Additional Personal Particulars Information โ€” is a shorter, more focused document. At four pages, it covers the essentials of character assessment: identity, travel history (5 years rather than 10), criminal history, and immigration history.

Form 1221 is typically requested for temporary visa applications or as a supplementary document during processing of certain applications. It does not ask for the depth of personal history that Form 80 requires.

Form 1221 asks about:

  • Identity and passport details
  • Overseas travel in the past 5 years
  • Criminal convictions and charges
  • Previous visa refusals and cancellations
  • Health conditions relevant to the character test

The key differences

Depth of personal history

Form 80 asks for your life history in far greater detail. If the Department needs to understand where you have lived, who you have worked for, your military background, and your organisational affiliations, it will request Form 80. Form 1221 only needs to establish the basics of your identity, recent travel, and character history.

Travel history window

Form 80 requires travel history for the past 10 years. Form 1221 only requires the past 5 years. If you are filling out Form 80, do not assume your travel history for years 6โ€“10 is not important โ€” it must be included.

When each is used

The Department decides which form to request based on your visa subclass and individual circumstances. Broadly:

  • Form 80 tends to be required for permanent visas, partner visas, humanitarian visas, and skilled visas where a full character assessment is needed
  • Form 1221 tends to be used for certain temporary visas, student visas in some cases, and as a supplementary document during ongoing processing

๐Ÿ’ก The Department does not publish a definitive mapping of visa subclasses to form types. Whether you need Form 80, Form 1221, or both is stated in your ImmiAccount or in correspondence from your case officer. Do not submit either form speculatively โ€” wait until it is specifically requested.

Can you be required to submit both forms?

Yes. It is possible โ€” and not uncommon โ€” to be asked to submit both Form 80 and Form 1221 at different stages of the same visa application. This can happen when:

  • The application was lodged with Form 1221, and during processing the case officer requests Form 80 for a more detailed assessment
  • The visa subclass instructions require both documents
  • Additional character checks are triggered by information in one of the forms

If you have already submitted Form 1221 and are subsequently asked for Form 80, ensure the information in both documents is consistent. Discrepancies between the two forms โ€” even minor ones involving approximate dates โ€” will attract scrutiny.

How do I know which one I need?

The definitive answer is in your ImmiAccount dashboard or your case officer's correspondence. If it is not clear:

  1. Check the visa application instructions for your specific subclass on the Department of Home Affairs website
  2. Contact your case officer directly and ask which form is required
  3. If you are working with a registered migration agent, they will know which form applies to your visa

Do not guess. Submitting the wrong form does not fulfil your obligation โ€” you will still be required to submit the correct one โ€” and submitting unnecessary forms can create inconsistencies that complicate your application.

Frequently asked questions

My visa instructions mention "Form 80 or Form 1221" โ€” which should I submit?

Some visa instructions list both forms as possibilities, with the actual requirement determined by your case officer based on your individual circumstances. Wait for specific direction from your case officer or ImmiAccount. If the instruction is unclear, contact the Department directly to ask which form applies to your case.

I filled out Form 1221 but my case officer is now asking for Form 80. Do I need to repeat all the same information?

Yes. Form 80 and Form 1221 are separate documents with different formats. You will need to complete Form 80 in full. Ensure the information in your Form 80 is consistent with what you submitted in Form 1221.

Is there a fee for submitting Form 80 or Form 1221?

No. Both forms are submitted as part of your visa application and there is no separate fee for either form.

Can I use FormMate 80 for Form 1221?

FormMate 80 is specifically designed for Form 80. Form 1221 is a shorter form available directly from the Department of Home Affairs website and can typically be completed in the PDF itself without a guided tool.
