Before you start: Gathering your information before opening Form 80 (or FormMate 80) significantly reduces completion time and the chance of errors. Most applicants spend more time finding documents than filling the form itself.

Australia's Form 80 Personal Character Assessment requires detailed personal history across 20 sections. Use this checklist to gather everything you need before you start. You can fill Form 80 online free once you have your information ready.

Passport details

  • Current passport โ€” passport number, issue date, expiry date, issuing country
  • All previous passports held in the past 10 years โ€” passport numbers and dates
  • Any travel documents other than a standard passport (e.g. refugee travel document)
  • Names exactly as they appear on each passport โ€” including any variations

Address history

  • Every address you have lived at since age 16 โ€” full street address, suburb, state/province, country
  • Exact or approximate dates for each address โ€” move-in and move-out dates
  • No gaps โ€” even temporary accommodation, student housing, living with relatives
  • Landlord or property manager names if available (some visa types request this)

Employment history

  • All employers โ€” company name, address, and type of business
  • Your position or job title at each employer
  • Start and end dates for each role
  • Periods of unemployment โ€” dates and reason (job seeking, study, parental leave, etc.)
  • Self-employment โ€” business name, ABN or equivalent, nature of work
  • Full-time study periods โ€” institution name, address, dates

Education history

  • All educational institutions from secondary school onward โ€” school/university name and address
  • Dates attended โ€” start and end dates for each institution
  • Qualifications obtained if applicable

Travel history

  • All international travel in the past 10 years โ€” every country visited
  • Approximate dates for each trip โ€” departure and return dates
  • Purpose of travel for each trip โ€” tourism, work, study, family visit, etc.
  • Transit stops longer than 24 hours โ€” even if you did not leave the airport
  • Passport stamps and boarding passes โ€” useful for verifying dates you cannot recall

Family details

  • Partner or spouse โ€” full name, date of birth, country of birth, country of citizenship, country of residence
  • Children โ€” names, dates of birth, countries of citizenship and residence (up to 6)
  • Parents โ€” names, dates of birth, countries of birth and citizenship (up to 4)
  • Siblings โ€” names, dates of birth, countries of citizenship (up to 5)
  • Previous relationships โ€” dates of marriage, separation, divorce for each

Character and criminal history

  • Any criminal charges, convictions, or sentences in any country โ€” regardless of how minor
  • Spent convictions โ€” must still be disclosed regardless of spent status in the country of origin
  • Dismissed charges โ€” must be disclosed even if the charge did not result in a conviction
  • Dates, offence type, and outcome for each matter
  • Court or jurisdiction details if available

Military or service history

  • Any service in armed forces in any country โ€” name of force, rank, dates of service
  • Reason for discharge if applicable
  • If you have never served โ€” mark "No" explicitly; do not leave blank

Visa and immigration history

  • All previous visas โ€” Australia and other countries, visa type, dates, grant numbers if available
  • Any visa refusals or cancellations โ€” in any country, with reasons if known
  • Any deportations or removals from any country
  • Any bans or exclusion orders from any country

Supporting documents

  • All current and expired passports โ€” to verify names, dates, and travel history
  • Bank statements โ€” useful for verifying travel dates and periods abroad
  • Employment contracts or payslips โ€” to confirm employer names, addresses, and dates
  • Tax records โ€” can help verify employment and address history
  • Tenancy agreements โ€” to confirm addresses and dates
  • Birth certificates โ€” for family member details
  • Marriage or divorce certificates โ€” if applicable
