Why 191 visa applicants search for Form 80
The Subclass 191 (Permanent Residence — Skilled Regional) visa is a permanent visa granted to applicants who have held a Subclass 491 or Subclass 494 provisional visa for at least three years, met the required income threshold, and complied with the obligation to live and work in a designated regional area during that time.
Because 191 applicants have already been through a rigorous character and skills assessment at the 491 or 494 stage, they sometimes assume that Form 80 will not be required again. In practice, the Department assesses each visa application independently. Some 191 applicants will be asked to provide Form 80 or Form 1221; others will not. The request depends on individual circumstances and case officer discretion.
If your circumstances have changed since your 491 or 494 application — for example, if you have acquired new criminal history, lived in additional countries, or had prior visa issues — the likelihood of a character form request may be higher. Always check your ImmiAccount correspondence for any document requests.
Form 80 vs Form 1221 for a 191 visa
The Department may request either Form 80 or Form 1221, depending on the circumstances of your application:
- Form 80 (Personal Character Assessment) is a comprehensive 20-section form covering all aspects of your biographical history including address history, employment, travel, education, family details, and detailed character questions. It is typically required when a more thorough character assessment is needed.
- Form 1221 (Additional Personal Particulars Information) is a shorter supplementary form that covers biographical details. It is typically used where a lighter-touch supplementary check is required.
The Department's request will specify which form is required. Do not substitute one for the other. See the Form 80 vs Form 1221 guide for a detailed comparison of the two forms.
What to prepare
If you are asked to complete Form 80 for your 191 visa application, gather the following documents before you begin. Use the Form 80 checklist as your complete reference:
- All current and expired passports
- All residential addresses for the last 10 years, with move-in and move-out dates — including any addresses during your time on the 491 or 494 visa
- Complete employment history since your last visa, including all employers, addresses, roles, and exact dates
- International travel history for the past 10 years — note that regional visa conditions may have restricted travel, but all travel must still be declared
- Education history from secondary school onward
- Family details, including your sponsor if relevant to your visa
- Details of any criminal history in any country, regardless of whether it is spent
- Details of any prior visa applications, refusals, or cancellations in any country
Consistency with your 491 or 494 history
One of the most important considerations for 191 applicants completing Form 80 is consistency with the information previously provided during your 491 or 494 application.
The Department has access to your prior character assessment records. Any unexplained discrepancies between what you stated in your 491 or 494 Form 80 and what you state in your 191 Form 80 may prompt clarification requests or further scrutiny. Pay particular attention to:
- Employment dates and employer names — these must align with your tax records, employer nomination documents, and any skills assessment you provided at the 491 or 494 stage. Inconsistencies in employment dates are among the most common triggers for clarification requests at the 191 stage.
- Addresses — your address history must accurately reflect the time you spent living in your designated regional area. This is especially important for the 191 visa because regional residency compliance is a core eligibility criterion.
- Travel history — your declared travel must be consistent with any prior character assessment and with your passport entry and exit records. If you travelled overseas during your 491 or 494 period, ensure the dates are accurate.
If you genuinely need to correct an error from a prior application, note the correction clearly in Part T (Additional Information) and explain the discrepancy. Do not simply carry forward an earlier error — a transparent correction is significantly better than an inconsistency that appears intentional.
Tip: Before completing Form 80 for your 191 application, review the Form 80 or Form 1221 you submitted at the 491 or 494 stage. Use it as a reference to ensure your dates, employer names, and addresses are consistent where applicable.

Frequently asked questions
Is Form 80 always required for the 191 visa?
No. Form 80 is not automatically required for all 191 visa applicants. Whether it is requested depends on your individual character circumstances — such as criminal history, countries you have lived in, and prior visa history. Check your ImmiAccount correspondence for any document requests. Do not assume Form 80 is required unless it has been specifically requested.
I submitted Form 80 with my 491 application. Do I need to submit a new one for my 191?
If the Department has formally requested Form 80 as part of your 191 application, yes — you need to complete a current Form 80 that reflects your circumstances at the date you sign it. Update all addresses, employment, and travel to cover the period since your prior submission. Ensure the new Form 80 is consistent with your prior records wherever the information overlaps.
What is the difference between Form 80 and Form 1221 for the 191 visa?
Form 80 is a comprehensive 20-section character assessment form. Form 1221 is a shorter supplementary biographical form. The Department specifies which is required — do not substitute one for the other. See the Form 80 vs Form 1221 guide for a full comparison.
My address during the 491 period was in a regional area — how do I list this?
List your regional addresses exactly as they were, with accurate start and end dates. Accuracy here is important because regional residency compliance is a key eligibility requirement for the 191 visa. If you moved between addresses within your regional area, list each address separately with the correct dates.
