Why Form 80 is used in visa applications

Character assessment is a component of most Australian visa types under the Migration Act 1958 (Cth). Form 80 provides the Department of Home Affairs with a comprehensive personal history — covering addresses, travel, employment, family background, and character — to assist in completing that assessment.

Form 80 is more comprehensive than the shorter Form 1221 (Additional Personal Particulars Information). Where Form 1221 covers a narrower set of personal history questions, Form 80 covers 20 sections and may be requested when the Department needs a more detailed picture of an applicant's background.

What Form 80 asks for

Form 80 covers 20 sections (Parts A through T) across a comprehensive range of personal particulars:

  • Parts A–C: Identity, other names, passport details
  • Part D (Q17): Address history — every residential address for the last 10 years, with no gaps
  • Part E (Q18): Travel history — all international trips in the last 10 years
  • Part F (Q19): Employment history — all paid work, self-employment, unemployment, and study
  • Parts G–H: Education history from secondary school onward
  • Parts I–J: Family details — parents, siblings, spouse or partner, children
  • Parts K–N: Character and criminal history — charges, convictions, investigations in any country
  • Parts O–R: Military service, organisational affiliations, immigration history, contact details
  • Part S: Declaration — signed and dated

  • Employment dates must align with any sponsor nominations, tax records, payslips, or employment letters already in your application
  • Addresses must reflect where you actually lived — they may be checked against lease records, bank statements, or utility bills
  • Travel history must match the entry and exit stamps in your passport
  • Names must be consistent across all documents — use your exact legal name as it appears on your passport
