Form 80 Review Checklist Before Upload
In brief
Before uploading Form 80 to ImmiAccount, check for gaps in your address and employment history, verify that dates across sections are consistent, confirm employer names are in full, and sign the declaration in Part S. A 10-minute check before upload can prevent a follow-up request.

Identity details
Work through your completed PDF and verify the following identity details before upload:
- Full legal name matches current passport exactly
- Date of birth is correct
- Passport number and expiry date are correct
- All other names used are listed (maiden name, aliases, name changes)
- All nationalities are listed
Address history
Gaps in address history are one of the most common reasons Form 80 is returned or queried. Check each of the following carefully:
- No gaps between consecutive address entries
- All entries include full street address, suburb, state/country
- All entries include start and end dates
- Temporary accommodation (share houses, couch surfing, living with family) is included
- Current address is listed as ongoing or with the correct end date
See Form 80 address history for guidance on gaps and overseas addresses.
Travel history
Cross-check your travel entries against your passport stamps and any booking records before uploading:
- All international trips from the last 10 years are listed
- Each trip has departure and arrival dates
- Each trip has a purpose (tourism, work, family visit, study, transit, etc.)
- Dates match passport stamps where possible
- No travel entry overlaps inconsistently with an Australian address entry
Employment history
Employment gaps and abbreviated employer names are among the most common errors that delay processing:
- All employer names are written in full (not abbreviated)
- All employment periods have start and end dates
- All gaps between jobs are explained (unemployed, study, carer, travelling, etc.)
- Self-employment uses the registered business name
- Current employment is listed as ongoing or with the correct end date
See Form 80 employment history for guidance on abbreviations and gaps.
Family details
Review each family section to confirm completeness and accuracy:
- Partner or spouse full name, date of birth, country of birth and citizenship are correct
- All children are listed (up to the form's maximum)
- Parent details are listed
- Sibling details are listed (up to the form's maximum)
- Former relationships are listed with dates
Character questions
The character section requires particular attention before uploading:
- All criminal charges, convictions, sentences are disclosed — including overseas and minor matters
- Charges that were withdrawn, dismissed, or resulted in no conviction are still disclosed
- Part K questions are answered explicitly (Yes or No — not left blank)
- Military or peacekeeping service is disclosed if applicable
- Organisational affiliations are disclosed if applicable
⚠️ Non-disclosure of criminal history is treated seriously by the Department. If you are uncertain about what to disclose, consult a registered migration agent (MARN holder) before submitting.
File and upload checklist
Before uploading to ImmiAccount, check these final file and submission details:
- Declaration in Part S is signed and dated
- Completed PDF file size is within ImmiAccount upload limits (typically under 5 MB)
- File is named clearly (e.g., Form80_YourName_Date.pdf)
- Uploading to the correct document request in ImmiAccount
- Keeping a copy of the uploaded file with the date you submitted it
See Where to upload Form 80 in ImmiAccount for step-by-step upload instructions.

Frequently asked questions
What should I check before uploading Form 80?
The most important checks are: no gaps in address history, no gaps in employment history, consistent dates across all sections, employer names in full, Part K character questions answered explicitly (Yes or No), and Part S declaration signed. Review the checklists on this page section by section before uploading.
What happens if I upload Form 80 with a gap in my address history?
Gaps in address history are one of the most common reasons a case officer requests a corrected or resubmitted Form 80. If a gap is identified, you may be asked to provide additional information, which can delay processing. It is better to review carefully and resubmit a corrected form than to upload an incomplete one.
Do I need to sign Form 80 before uploading?
Yes. Part S of Form 80 is the declaration section. It must be signed and dated before the form is complete. Uploading an unsigned Form 80 may result in a request to resubmit.
Where do I upload Form 80 in ImmiAccount?
Upload your completed Form 80 PDF against the specific document request in your ImmiAccount application. If Form 80 has been formally requested, a document request line should appear in your application. If you cannot find it, see the ImmiAccount upload guide for step-by-step instructions.
Is FormMate 80 affiliated with the Australian Government?
No. FormMate 80 is an independent tool. It is not affiliated with, endorsed by, or connected to the Australian Government or the Department of Home Affairs. The official Form 80 PDF is available at immi.homeaffairs.gov.au.
